TLDR Chronic burn scars may lead to scalp skin issues, so doctors should consider this when diagnosing.
The document reports on three cases of erosive pustular dermatosis of the scalp (EPDS) that developed on long-standing burn scars in Asian women aged 57, 66, and 79. This rare inflammatory condition, characterized by pustules, erosions, and crusts, can lead to scarring alopecia and is challenging to diagnose and manage, especially on burn scars. The study suggests that burn scars may trigger abnormal autoimmune reactions due to their susceptibility to injury and decreased hair follicle density. Treatments such as topical clobetasol propionate, oral medications, and systemic isotretinoin led to improvement or stabilization of lesions. The authors emphasize the importance of recognizing EPDS in patients with a history of scalp trauma to provide timely treatment and prevent complications, and they call for further research on the association between hair follicle destruction and EPDS pathogenesis.
