TLDR Erythema multiforme-like lesions can occur in lupus without drug use or herpes infection.
In this case study, a 17-year-old woman with systemic lupus erythematosus (SLE) presented with rare erythema multiforme-like lesions on her hands and feet, alongside symptoms of photosensitivity and hair loss. Histopathological and immunofluorescence examinations confirmed the diagnosis of SLE, revealing hyperkeratosis, parakeratosis, and positive lupus band test, among other findings. Treatment with methylprednisolone led to significant improvement in her symptoms and lesion appearance. This case highlighted that erythema multiforme-like lesions, although uncommon, can occur in systemic lupus erythematosus without a history of drug use or herpes virus infection.
