Dermoscopy of Annular Elastolytic Giant Cell Granuloma

    TLDR Dermoscopy helped diagnose a rare skin disease which slightly improved with treatment.
    The document reports a case of annular elastolytic giant cell granuloma (AEGCG), a rare skin disease, in a 46-year-old female patient who presented with a 2-month history of an enlarging frontal alopecic plaque. Dermoscopy revealed multiple yellowish/orange follicular dots in a diffuse erythemato-whitish background. Histopathological examination confirmed the diagnosis of AEGCG, showing a granulomatous infiltrate with multinucleated giant cells and elastic fibers in the cytoplasm of these cells. The patient was treated with a medium potency topical corticosteroid and tacrolimus 0.1% ointment, with mild benefit. After 6 months, intralesional steroids were administered, resulting in some improvement in the erythema and stability of the size of the lesions. The authors suggest that the dermoscopy findings could help in the differential diagnosis of scalp dermatosis with yellowish-orangish dots.
