TLDR A woman was diagnosed with Sheehan's syndrome and a rare complication of diabetes insipidus 33 years after symptoms began, improving with hormone therapy.
A 58-year-old woman was diagnosed with delayed onset Sheehan's syndrome (SS) 33 years after childbirth, complicated by diabetes insipidus (DI). She presented with symptoms of hypothyroidism, fatigue, and hair loss, and had not lactated or menstruated since her last childbirth. Examination revealed panhypopituitarism and an MRI showed a partially empty sella turcica. Treatment with prednisone and levothyroxine improved her symptoms, but unmasked DI, which was managed with desmopressin. This case highlights that SS can occur many years postpartum and may be underdiagnosed in developed countries, especially among migrant women. Early diagnosis and treatment are crucial for improving quality of life and reducing morbidity and mortality.
