This case report details a rare instance of a primary cutaneous granular cell tumor (GCT) with overlying hypertrichosis in a 27-year-old woman, marking the first such case in an adult. The patient exhibited a firm, pigmented plaque on her upper back with increased hair growth. Diagnosis was confirmed through histopathologic examination and immunohistochemical staining, showing large polygonal cells with granular eosinophilic cytoplasm. The lesion was surgically excised, revealing no malignancy. This case underscores the importance of recognizing rare presentations of GCTs, as they can mimic other conditions, and highlights the need for clinicopathologic correlation in diagnosis.
