TLDR Creatine does not cause hair loss.
This 12-week randomized controlled trial with 38 resistance-trained males aged 18-40 found no significant differences in dihydrotestosterone (DHT) levels, DHT-to-testosterone ratio, or hair growth parameters between the creatine and placebo groups. The study provides strong evidence against the claim that creatine supplementation (5 g/day) contributes to hair loss, as there were no changes in hair count, density, or follicle quality. Additionally, creatine was found to be safe, with no changes in creatinine or estimated glomerular filtration rate (eGFR) observed.
