Clomiphene Citrate Versus Letrozole for Ovulation Induction in PCOS: A Comparative Study

TLDR Letrozole is better for single follicle development and endometrial thickness in PCOS, with no significant difference in pregnancy rates compared to clomiphene.
This study compared clomiphene citrate and letrozole for ovulation induction in 200 women with PCOS. Clomiphene resulted in a higher number of follicles, but letrozole showed better monofollicular development and endometrial thickness. There was no significant difference in dominant follicle size, ovulation rate per cycle, or pregnancy rates between the two groups. Although the number of pregnancies was slightly higher in the letrozole group, it was not statistically significant. Notably, clomiphene led to one quadruplet and one twin gestation, while letrozole did not result in multiple gestations. No cases of ovarian hyperstimulation syndrome or congenital anomalies were reported in either group.
