The document reports a case of cicatricial female pattern hair loss in a 36-year-old woman who had been experiencing gradually worsening hair loss for over 10 years. Skin and TrichoScan examinations revealed diffuse hair thinning, increased proportions of vellus hairs, and areas of focal atrichia. Histopathological examination showed miniaturized hair follicles, lichenoid folliculitis, perifollicular fibrosis, and a significant decrease in the number of hair follicles compared to healthy individuals of the same age. The patient was treated with topical 5% minoxidil liniment, tacrolimus ointment, and clobetasol propionate ointment, as well as oral spironolactone (20 mg twice a day) and compound glycyrrhizin capsules (50 mg three times a day). After six months of treatment, there was no significant worsening of hair loss.
