    Polycystic ovarian syndrome (PCOS) affects 6-20% of reproductive women globally and is characterized by metabolic, reproductive, and psychological issues. Despite its prevalence, there is no cure, and treatments only manage symptoms due to limited understanding of its causes. This review highlights that hyperandrogenism may be a causative factor in PCOS, not just a symptom. It explores the mechanisms of androgen synthesis, conversion, and metabolism, emphasizing the role of adrenal-derived 11-oxygenated androgen. The review also discusses how androgen receptor signaling contributes to PCOS traits and suggests therapeutic strategies targeting hyperandrogenism.
