Histopathological examination confirmed the diagnosis, revealing a cyst wall lined by stratified squamous epithelium with dense laminated keratin and dystrophic calcification. Trichilemmal cysts, typically found in hairy regions like the scalp and face, are uncommon in the eyelid, especially in children, and can be misdiagnosed as chalazion. This case highlights the importance of considering trichilemmal cysts in atypical eyelid lesions in children.
