TLDR A fungal infection developed on a herpes scar and was successfully treated with antifungal medication.
The document described a case study of a 37-year-old man who initially presented with herpes zoster, which was treated successfully with oral acyclovir. Five months later, he developed a pustular eruption in the same area, initially suspected to be recurrent herpes zoster or folliculitis. Methicillin-resistant Staphylococcus aureus was isolated, but treatment with antibiotics showed no improvement. Histopathologic examination revealed eosinophilic pustular dermatosis, and further investigation identified Trichophyton rubrum from a fungal culture. The patient was treated with itraconazole pulse therapy and cycloproxolamine cream, leading to significant improvement, with complete resolution of pustules after 5 months.
