TLDR Trichofolliculomas can sometimes be more aggressive than thought.
Trichofolliculoma, a rare benign tumor of hair follicle origin, was typically non-invasive, with 59 previously recorded cases showing no malignancy. However, this report from 1979 described a unique case involving a 57-year-old man with a trichofolliculoma on his cheek that exhibited perineural invasion, a feature not previously documented in such tumors. The tumor, measuring 1.6 cm, was surgically excised and showed a central keratin-filled cyst with squamous epithelium. This case raised clinical concerns about the potential for malignancy in trichofolliculomas, suggesting a need for careful monitoring and consideration of trichofolliculocarcinoma as a possible diagnosis.
