TLDR Tinea capitis should be considered in adults with scalp issues for proper treatment.
This case report details a 75-year-old immunocompetent woman with tinea capitis caused by Trichophyton violaceum, a rare condition in adults. Initially misdiagnosed and treated with ineffective antibiotics, the correct diagnosis was achieved through fungal culture and microscopy. Treatment with high-dose itraconazole and prednisolone led to complete lesion clearance and satisfactory hair regrowth. A review of 11 similar cases from 2018 to 2023 underscores the need to consider tinea capitis in the differential diagnosis of inflammatory scalp lesions in adults, including those who are immunocompetent, to ensure timely and accurate treatment.
