TLDR Surgery may be needed for resistant thyrotoxicosis.
This document reported a case of refractory thyrotoxicosis in a 30-year-old patient, where conventional treatments such as antithyroid drugs, radioactive iodine, and adjunctive therapies like beta-blockers and corticosteroids were ineffective. The patient presented with a progressively enlarging neck swelling, symptoms of hyperthyroidism, and hair loss. Despite being on carbimazole for 3 years, the patient's thyroid function tests remained abnormal, indicating resistance to treatment. Ultimately, a total thyroidectomy was performed, revealing a hypervascular multinodular thyroid gland. This case highlighted the rare necessity of surgical intervention in managing resistant thyrotoxicosis.
