Case 1: Severe Acne – Not Just Skin Deep

TLDR Severe acne in a young girl may indicate underlying hormonal issues.
A 10-year-old girl with a three-year history of severe, treatment-resistant facial acne was evaluated by a pediatric dermatologist. She had early onset of pubic and axillary hair at seven years old and early breast development by nine, but was pre-menarcheal. Despite no significant growth spurt, she was notably tall for her age, matching her 15-year-old sister's height. Physical examination revealed extensive acne, advanced pubic and axillary hair growth, and Tanner stage 3 breast development. Her height and weight were above the 75th and 90th percentiles, respectively. Further investigations were conducted to understand her condition.
