TLDR Multiphoton microscopy can effectively distinguish between scarring and non-scarring alopecia.
The study explored the use of multiphoton microscopy (MPM) as a non-invasive method to image the scalp and distinguish between scarring and non-scarring types of alopecia. It involved 5 healthy subjects and 12 patients with different types of alopecia. MPM allowed for in vivo imaging of hair follicles and surrounding structures, identifying features such as sebaceous glands in non-scarring alopecia and miniaturized hair follicles with macrophages and lymphocytes in scarring alopecia. The study found that hair follicle diameters were significantly smaller in scarring alopecia compared to non-scarring alopecia and normal scalps, suggesting MPM could effectively differentiate between these conditions.

Pelage is developing a topical hair follicle stem cell therapy, PP405, for non-scarring alopecias like androgenetic alopecia, with Phase III trials planned and a potential market launch by 2027. The treatment may not require continuous use after initial regrowth.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.

Dutasteride and finasteride are not effectively stopping hair loss for some users, despite long-term use. Some are considering or using RU58841, pyrilutamide, and other treatments like microneedling, while also managing seborrheic dermatitis with diet and topical solutions.

community Topical Minoxidil is a pain in the ass for diffuse thinners

Topical minoxidil being a difficult product to apply, with users considering finasteride, RU58841, and oral minoxidil as alternative treatments for diffuse thinning hair. Other advice includes shaving the head in the first 3 months of using minoxidil and finding ways to apply it without saturating the hair.
