Primary Cutaneous Diffuse Large B-Cell Lymphoma of the Scalp: A Case Report and Brief Review of the Literature

    TLDR Lymphoma should be considered in unusual scalp hair loss cases, as treatment can work but may lead to severe complications.
    This case report describes a rare occurrence of primary cutaneous diffuse large B-cell lymphoma (PCDLBCL-NOS) on the scalp of a 45-year-old woman, presenting with atypical alopecic lesions. The patient achieved complete remission after six cycles of R-CHOP chemotherapy but later developed B-cell acute lymphoblastic leukemia and died from neutropenic sepsis despite HYPER-CVAD treatment. The report emphasizes the importance of considering lymphoma in the differential diagnosis of alopecic scalp lesions and the potential for severe hematologic complications. It also highlights the effectiveness of R-CHOP in early-stage disease and the need for timely biopsy, while noting limitations such as incomplete molecular profiling.
