TLDR Tight hairstyles under hijabs cause traction alopecia, worsened by prolonged wear.
This literature review on traction alopecia (TA) in hijab-wearing patients identifies tight hairstyles under hijabs as a primary cause of TA, exacerbated by prolonged wear. The review of 25 relevant articles from 2008 to 2023 highlights that the popular two-layer turban style contributes to follicular occlusion, transepidermal water loss, and high tension on hair follicles. Proper examination etiquette includes conservative exposure of affected areas with gender-concordant health teams. Clinical findings in hijab-wearing patients are similar to non-hijab-wearing patients, with the addition of a linear groove on the frontal scalp from undercap pressure. Management should include guidance on using silk and satin fabrics for hairstyling.
