Transient Renal and Hepatic Dysfunction in Severe Profound Hypothyroidism

The study reported on two pediatric cases of severe hypothyroidism that presented with transient renal and hepatic dysfunction. A 12-year-old male and a 15-year-old female both exhibited symptoms such as lethargy and growth failure, with the male also experiencing hair loss. Both patients had significantly elevated TSH levels and low Free T4 levels, indicating profound hypothyroidism. The male patient also had elevated creatinine, urea, ALT, and CPK levels, while the female had elevated creatinine. Upon treatment with thyroxine, both patients' liver and kidney functions normalized within 6 weeks. The study highlighted that severe hypothyroidism could lead to temporary liver and kidney issues, which resolved with appropriate thyroid hormone replacement therapy. Delayed diagnosis of hypothyroidism was suggested to contribute to these transient dysfunctions.
