A 20-year-old Mexican woman, initially misdiagnosed with polycystic ovarian syndrome, was found to have a rare ovarian steroid cell tumor, not otherwise specified (OSCT-NOS), which accounts for less than 0.1% of all ovarian neoplasms. This tumor was causing her symptoms of secondary amenorrhea and virilization, including acne, low-pitched voice, and androgenic alopecia. After a right salpingo-oophorectomy, her testosterone levels normalized, her menstrual cycle became regular, and there was no recurrence of virilization signs. This case emphasizes the need for physicians to consider rare tumors in young women with increased testosterone levels after ruling out common causes like polycystic ovarian syndrome.
