A rare case of tinea capitis potentially caused by Penicillium was reported in a 3-year-old boy who presented with patchy hair loss, loss of hair luster, and scalp crusts. Despite normal results from Wood's light and potassium hydroxide examinations, a culture revealed Penicillium species. The patient was treated with 50 mg of ketoconazole daily.
