Persistent Pemphigus Vulgaris and Pemphigus Foliaceus Showing Features of Tufted Hair Folliculitis on the Scalp

    TLDR A rare scalp condition with hair loss was correctly diagnosed and treated, leading to hair regrowth.
    This case report described a 52-year-old female patient with a rare combination of pemphigus vulgaris (PV) and pemphigus foliaceus (PF) localized to the scalp, presenting with painful crusted erosions and hair loss, but no mucosal involvement. Initially misdiagnosed as psoriasis, the correct diagnosis was made through biopsy and direct immunofluorescence, revealing features of tufted hair folliculitis. Treatment with systemic and topical corticosteroids led to clinical remission and regrowth of scalp hair. This case highlighted the unusual presentation of both PV and PF on the scalp, which had not been previously reported.
