A 5-month-old Brittany spaniel developed severe skin issues after being treated with cephalexin for cystitis, presenting with painful crusted lesions and systemic symptoms. Initial diagnosis suggested erythema multiforme, but later symptoms and histopathology indicated pemphigus foliaceus, likely triggered by the drug. Treatment with prednisolone, antiseptic baths, and antibiotics led to significant improvement, with complete resolution of skin lesions and no relapse for 4 years. This case highlighted the progression from erythema multiforme to pemphigus foliaceus as a drug-induced reaction.
