    This case report details an 18-year-old female misdiagnosed with polycystic ovary syndrome (PCOS) due to overlapping symptoms with an ovarian steroid cell tumor (OSCT), such as irregular menstrual cycles and hyperandrogenism. Standard PCOS treatments were ineffective, prompting further investigation that revealed the OSCT. Surgical removal of the tumor led to symptom improvement and confirmed the diagnosis. The report underscores the importance of considering OSCT in differential diagnoses for hyperandrogenism and highlights the need for comprehensive testing to avoid misdiagnosis, emphasizing early identification and management for better patient outcomes.
