TLDR Ovarian hyperthecosis can cause high testosterone and symptoms like hair loss, and surgery can help improve these symptoms.
This case study discusses a 62-year-old woman with ovarian hyperthecosis (OH), a rare cause of hyperandrogenism, characterized by elevated testosterone levels and symptoms like hair loss and hirsutism. Despite normal-sized ovaries on imaging, the patient had an enlarged ovarian cyst and underwent bilateral salpingo-oophorectomy. Histopathology confirmed OH, and postoperative tests showed normalized testosterone levels, with gradual improvement in hirsutism and hair loss. The study emphasizes considering OH in severe hyperandrogenism cases and highlights oophorectomy and histopathologic diagnosis as the gold standard for management.
