This systematic review of mixed epithelial and stromal tumor of the seminal vesicle (MESTSV) covers 80 years of literature and identifies 23 cases, providing a comprehensive clinical, histologic, and immunohistochemical characterization. MESTSV is a rare, biphasic neoplasm with abundant stromal proliferation, architectural heterogeneity, and strong immunoreactivity for estrogen and progesterone receptors. Although most cases are benign, the presence of stromal nuclear atypia suggests the need for long-term surveillance with serial imaging to monitor for recurrence or metastasis. The review also includes a novel case report of a 38-year-old male with a large pelvic tumor, highlighting the clinical presentation and surgical findings.
