This case report details a rare instance of melanotrichoblastoma, a pigmented variant of trichoblastoma, marking only the seventh documented case. A 51-year-old female presented with a lesion in the pelvic girdle, a novel location for this tumor type. Histopathological and immunohistochemical analyses confirmed the diagnosis, distinguishing it from basal cell carcinoma and melanoma. Despite being benign, the tumor's rarity requires regular follow-up, with no recurrence observed at eight months. The report emphasizes the importance of distinguishing melanotrichoblastoma from other pigmented skin tumors and suggests its inclusion in future WHO classifications to prevent misdiagnosis.
