Folliculitis decalvans (FD) is a rare inflammatory scalp disease causing scarring alopecia, primarily affecting young and middle-aged men. This report details a 20-year-old male with FD, presenting with painful pustules on the scalp. The condition, potentially linked to Staphylococcus aureus and influenced by genetic and immunological factors, was treated successfully with a month-long regimen of doxycycline and topical ozenoxacin, resulting in remission of the lesions.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.
