    This case report details a 24-year-old Palestinian female with hirsutism and irregular menstrual cycles, diagnosed with a rare malignant ovarian Leydig cell tumor. Initial treatments were ineffective, and the disease progressed to stage four with metastasis despite surgery and chemotherapy. The report emphasizes the diagnostic challenges and aggressive nature of these tumors, particularly in young women, and the necessity of multidisciplinary management. The patient's condition deteriorated, highlighting the importance of histopathological examination and team discussions in managing such complex cases.
