A Case of Simultaneously Occurring Lichen Sclerosus and Segmental Vitiligo: Connecting the Underlying Autoimmune Pathogenesis

    This case study described a 41-year-old African-American woman who simultaneously experienced segmental vitiligo and lichen sclerosus. She presented with asymptomatic white macules and patches on her left foot, spreading to her thigh and buttocks, and noted depigmented pubic hair on the left side. Additionally, she experienced severe burning and itching in the vaginal area, with whitish pink skin changes. A biopsy confirmed lichen sclerosus, and histology showed absence of melanin in the epidermis, consistent with vitiligo. Treatment with clobetasol ointment 0.05% led to some repigmentation and symptom resolution in the genital area over 6 weeks, although slight scaling persisted. This case highlighted the potential autoimmune connection between the two conditions.
