A Comparative Clinical Study of Intralesional Platelet-Rich Plasma and Intralesional Corticosteroid in Managing Alopecia Areata: Efficacy and Patient Acceptability

Prabhat Kumar, Varsha Rani, Samiksha Nayak
TLDR PRP is a promising alternative for alopecia areata due to its safety and patient satisfaction.
This study compared the efficacy and patient acceptability of intralesional platelet-rich plasma (PRP) versus intralesional corticosteroids (ILCs) in managing alopecia areata in 30 patients. Both treatments significantly improved hair regrowth, with PRP showing a slower initial response but comparable results at 12 weeks. PRP had higher patient satisfaction despite injection discomfort, while ILCs offered faster regrowth but more side effects like skin atrophy. Both treatments were effective, but PRP's favorable safety profile and patient satisfaction suggest it as a promising alternative for long-term management or for those at risk of steroid-related side effects.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.
