This case report highlights the adverse effects of methotrexate toxicity and underscores the importance of early recognition of its symptoms. It details the case of a 54-year-old female with mixed connective tissue disease who mistakenly took high doses of methotrexate for five consecutive days instead of the prescribed weekly dose. Initially asymptomatic, she was discharged from the ER with normal CBC values. However, upon further evaluation by her primary care physician, she developed mucocutaneous lesions, oral and throat pain, and pancytopenia, leading to her hospital admission for treatment of methotrexate toxicity.
