High-Dose Vitamin D3 and Tonsillectomy as Therapeutic Management in Henoch-Schönlein Purpura Following Hepatitis B Vaccination: A Rare Case Report

    TLDR Hepatitis B vaccination can trigger HSP, and vitamin D correction and tonsillectomy can effectively manage it.
    A 19-year-old woman developed Henoch-Schönlein purpura (HSP) following a hepatitis B vaccination, presenting with a purpuric rash and joint pain. Initial treatment with mycophenolate mofetil, methylprednisolone, vitamin D3, and folic acid was only partially effective. Adjustments to her medication regimen and the addition of fexofenadine improved her condition over 3 months, but she experienced knee pain and hair loss. A tonsillectomy in May 2021 led to complete resolution of the rash and normalization of vitamin D levels. This case suggests that hepatitis B vaccination can trigger HSP and highlights the importance of vitamin D correction and tonsillectomy in managing the condition.
