TLDR Hepatitis B vaccination can trigger HSP, and vitamin D correction and tonsillectomy can effectively manage it.
A 19-year-old woman developed Henoch-Schönlein purpura (HSP) with a purpuric rash and joint pain following a hepatitis B vaccination. Despite initial treatment with mycophenolate mofetil, methylprednisolone, vitamin D3, and folic acid, her symptoms persisted. Adjustments to her medication regimen and the addition of fexofenadine led to some improvement, but she continued to experience symptoms, including knee joint pain and hair loss. After undergoing a tonsillectomy for chronic tonsillitis, her rash resolved completely, and her vitamin D levels normalized. This case suggests that hepatitis B vaccination can trigger HSP and highlights the potential benefits of vitamin D correction and tonsillectomy in managing the condition.
