An Interesting Case of Hepatic Adrenocortical Carcinoma

    This document reported the first known case of hepatic adrenocortical carcinoma (ACC) in a 21-year-old Sundanese woman who presented with virilization symptoms, including hirsutism, deepening voice, and alopecia. Hormonal evaluations revealed significantly elevated levels of free testosterone, dehydroepiandrosterone sulfate, and estradiol, with low levels of LH and FSH. Imaging showed a large mass in the right liver lobe, and liver biopsy confirmed ACC. Despite treatment with spironolactone, there was no significant symptom improvement. The case highlighted the importance of thorough clinical evaluation and the potential for ectopic ACC to present similarly to adrenal gland tumors, with surgical resection being the primary treatment approach.
