    Hairline-lowering surgery (HLS) is an effective method for reducing a high hairline, achieving an average reduction of 2 cm, equivalent to 3,000 hair grafts. This study presents a case of a 46-year-old female who underwent HLS using bone-tunneling suture fixation, resulting in a 1.5 cm reduction in forehead length. The procedure involved a trichophytic incision, scalp flap advancement, and bone tunneling for secure fixation. Postoperative results showed no complications, and the patient and surgeons were satisfied with the outcome. HLS offers immediate and noticeable results, making it a preferable alternative to hair grafting for patients with high hairlines.
