TLDR A woman developed folliculitis after long-term acitretin treatment for pustular psoriasis.
A 37-year-old woman with recalcitrant pustular psoriasis developed folliculitis, a rare side effect, after nearly 1 year of treatment with acitretin (50 mg/day). Despite partial resolution of psoriasis lesions with acitretin, she presented with pruritic papules on her anterior thigh and forearms. The folliculitis was successfully treated with skin dressing and antibiotics. This case highlighted the need to consider folliculitis in the differential diagnosis of papular eruptions in patients on high-dose acitretin.
