TLDR A woman developed folliculitis after long-term acitretin treatment for pustular psoriasis.
A 37-year-old woman with recalcitrant pustular psoriasis developed folliculitis, a rare side effect, after nearly 1 year of treatment with acitretin (50 mg daily). Despite partial resolution of psoriasis with acitretin, she presented with pruritic papules on her thighs and forearms. The folliculitis was successfully treated with skin dressing and antibiotics. This case highlighted the importance of considering folliculitis in the differential diagnosis of papular eruptions in patients on high-dose acitretin.

Pioglitazone is suggested for Lichen Planopilaris, while Omega-3s and reducing processed foods may improve sebum quality, though genetic factors play a significant role.
