Flagellate Shiitake Mushroom Reaction With Histologic Features of Acute Generalized Exanthematous Pustulosis

    TLDR Eating shiitake mushrooms caused a man to develop a rash similar to a skin condition known as AGEP.
    A 59-year-old man developed a severely pruritic rash with flagellate, linear red patches and small pustules after consuming shiitake mushrooms daily for three days. He had a history of type 2 diabetes and end-stage renal disease, among other conditions, and was on multiple medications. Physical examination showed diffuse erythematous patches on various parts of his body. A shave biopsy from a pustule indicated spongiotic dermatitis. This case suggests a flagellate shiitake mushroom reaction with histologic features resembling acute generalized exanthematous pustulosis (AGEP).
