Adjunctive Dupilumab Therapy in a Partial Responder to Baricitinib for Alopecia Areata

TLDR Adding dupilumab led to almost complete hair regrowth in a patient with alopecia areata who only partially responded to baricitinib.
This article discusses a case of severe alopecia areata with a predominant ophiasis pattern, where the patient partially responded to baricitinib 4 mg daily and oral minoxidil over 15 months. However, after the addition of dupilumab, the patient experienced near complete hair regrowth within just 2 months. This case highlights the potential of using combination immunomodulatory agents, such as dupilumab, to enhance treatment responses in refractory cases of alopecia areata, especially when initial treatments with JAK inhibitors like baricitinib do not achieve desired outcomes.
