TLDR Severe digestive issues in DRESS need early endoscopy for better treatment.
The document described two cases of Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS) with severe digestive tract involvement, which had not been previously reported. The first case involved a 70-year-old woman treated with leflunomide, who developed a rash, fever, and severe digestive symptoms, ultimately leading to her death from a massive digestive hemorrhage. The second case was a 28-year-old woman treated with lamotrigine, who also developed a rash, fever, and severe digestive symptoms, but achieved complete remission with local corticosteroids. These cases highlighted the importance of recognizing and managing severe digestive involvement in DRESS, suggesting that endoscopy should be conducted when digestive symptoms appear to guide more aggressive treatment.
