The Occurrence of Discoid Lupus Erythematosus Progressing to Systemic Lupus Erythematosus, with Overlapping ANCA-Associated Vasculitis, and Rheumatoid Arthritis Presenting as Papular-Purpuric Glove and Sock Syndrome

This case report discusses a 28-year-old female with a history of papular-purpuric glove and sock syndrome (PPGSS) who progressed from discoid lupus erythematosus (DLE) to systemic lupus erythematosus (SLE), with overlapping rheumatoid arthritis (RA) and ANCA-associated vasculitis (AAV). Despite the rarity of such overlap syndromes, especially in the discoid subtype, the patient exhibited symptoms like fever, rash, alopecia, and joint pain, meeting the ACR/EULAR 2019 Criteria for SLE. Treatment with corticosteroids and DMARDs resolved most symptoms, though discoid lesions remain. This case underscores the importance of recognizing potential overlap syndromes in resource-limited settings like the Philippines.
