A 25-year-old transgender woman with severe plaque psoriasis developed generalized hypertrichosis after starting ciclosporin, despite being on testosterone-blocking therapy with estradiol and triptorelin. The hypertrichosis affected her arms, fingers, and face, causing distress and leading to the discontinuation of ciclosporin. She was switched to adalimumab, which controlled her psoriasis without causing hypertrichosis. This case highlights the potential for ciclosporin to induce hypertrichosis even with androgen suppression, marking the first such documented instance in a transgender woman. Clinicians should be aware of this side effect and its psychological impact, and further research is needed to understand the influence of sex hormones on this condition.
