TLDR Cryptococcoid Sweet syndrome can be linked to hydralazine use and multiple autoantibodies, requiring high clinical suspicion and more research.
A man in his 50s with a complex medical history, including hypertension, diabetes, and past substance use, developed acute renal failure, respiratory failure, and a painful rash. Diagnosed with cryptococcoid Sweet syndrome, a rare variant of Sweet syndrome, he was treated with methylprednisolone and dapsone, leading to gradual resolution of symptoms. This case, along with a literature review, highlights the association of this syndrome with hydralazine use and the presence of multiple autoantibodies. The review emphasizes the need for high clinical suspicion and further research into this rare condition.
