Congenital Lipedematous Alopecia in a 6-Year-Old Asian Boy

    This case report describes the first known instance of congenital lipedematous alopecia (LA) in a male, specifically a 6-year-old Asian boy. LA is characterized by scalp thickening and non-scarring alopecia, with a significant female preponderance. The boy exhibited patchy hair loss since birth, with a soft, spongy scalp texture and localized telangiectasia. Histopathology showed thickened subcutaneous adipose tissue and reduced hair follicles. Despite no medical treatment, minor improvement was observed over 3 years. The pathogenesis of LA is unclear, though genetic factors and sex hormones may play roles. LA should be considered in congenital alopecia cases, but effective treatments remain elusive.
