This study conducted comprehensive genomic profiling on 276 cases of cutaneous adnexal carcinomas (CACs), revealing distinct genomic characteristics across different tumor types. Sebaceous gland tumors showed the highest number of genetic alterations, including TP53 and RB1 mutations, and exhibited high tumor mutational burden (TMB) and microsatellite instability-high (MSI-H) status, suggesting potential responsiveness to immunotherapy. Sweat gland tumors, particularly eccrine, displayed unique gene alterations. The study emphasizes the heterogeneity of CACs and the importance of molecular profiling in guiding personalized treatment strategies, highlighting the need for further clinical trials to explore the prognostic and predictive value of CAC-specific biomarkers for immune checkpoint inhibitor response.
