TLDR Finasteride modifies 5-alpha-reductases through a two-step process, affecting inhibitor potency and possibly causing side effects.
This study examines the chemical mechanism of the covalent modification of 5-alpha-reductases by finasteride. The study suggests a two-step mechanism involving a high barrier addition of the attacking nucleophile at C-1, followed by a low barrier protonation at C-2. The proposed mechanism has implications for the design of novel covalent inhibitors for use as pharmaceutics. The study also notes that the partitioning of the activated intermediate, which occurs in the mechanism-based inhibition, undermines the potency of the inhibitor and possibly causes adverse effects in vivo.

A 15-year-old is concerned about hair loss, possibly at Norwood 2 or 3, and is using shampoos and conditioners recommended by a trichologist. Suggestions include considering topical minoxidil and consulting a doctor about topical anti-DHT treatments like finasteride or RU58841, but avoiding 5-alpha-reductase inhibitors at this age.

The user shared their experience with Finasteride and Dutasteride for hair loss. They initially had success with Finasteride but switched to Dutasteride after experiencing shedding, which continued, and are now considering a hair transplant.

community A hill on which I will die: The DHT itch absolutely exists

      in Research/Science  210 upvotes 1 year ago
The DHT itch is linked to hair loss and persists despite finasteride use; switching to dutasteride helped alleviate the itch and promoted regrowth. Some users suggest seborrheic dermatitis as a cause and recommend treatments like medicated shampoos, vitamin D, and minoxidil.
