TLDR Treatments for enlarged prostate can cause problems with ejaculation, and less invasive options might preserve sexual function better.
The document reviews the impact of treatments for benign prostatic hyperplasia (BPH) on ejaculatory function. It reveals that most BPH treatments carry significant risks of ejaculatory dysfunction. Pharmacologic treatments like Tamsulosin, Alfuzosin, Silodosin, Finasteride, and Dutasteride have varying degrees of impact on ejaculation, with Silodosin showing the highest rates of abnormal ejaculation (28.1%). Surgical treatments like transurethral resection of the prostate (TURP) and holmium laser enucleation of the prostate (HoLEP) were associated with significant declines in ejaculatory function, with up to 65% of men experiencing dysfunction after TURP. Minimally invasive techniques like prostatic urethral lift (PUL) and water vapor thermal therapy are suggested to offer the highest chance for preserving sexual function. The document concludes that treatments for BPH can significantly impact ejaculatory function and suggests that alternative treatments may preserve ejaculatory function, but further studies are needed to confirm their efficacy and safety.
