In 2020, a rare case of bilateral extensive nevus comedonicus of the eyelids was reported, which led to complications including bilateral ptosis and ectropion of the lower eyelids. The patient, who first exhibited lesions at the age of 35, underwent blepharoplasty on both upper eyelids. Histopathological examination of a skin biopsy revealed the characteristic large, grouped, dilated follicular ostia filled with keratin. This case was notable for the late onset of the condition and the uncommon location on both eyelids.
