TLDR Consider DRIF and perform skin biopsies for persistent papular rashes.
A 45-year-old African American male with chronic allergic rhinitis and atopic dermatitis presented with intense burning and pruritis, and widespread papules. Despite extensive evaluations and various treatments, his condition remained undiagnosed until a skin biopsy confirmed disseminated and recurrent infundibulofolliculitis (DRIF). This rare condition, often seen in young African American males, did not respond well to antihistamines and topical steroids. The patient was transitioned to isotretinoin following the biopsy. The study highlighted the importance of considering DRIF in similar cases and the benefit of skin biopsies for accurate diagnosis.
