The Atypical Itch That Rashes: Disseminated And Recurrent Infundibulofolliculitis (DRIF) In An Atopic African American Male

    A 45-year-old African American male with chronic allergic rhinitis and atopic dermatitis presented with intense burning and pruritis, and widespread papules. Despite extensive evaluations and various treatments, including antihistamines, steroids, and phototherapy, no diagnosis was found until a skin biopsy revealed disseminated and recurrent infundibulofolliculitis (DRIF). The patient was then treated with isotretinoin. DRIF is a rare condition, often seen in young African American males, and does not respond well to typical atopic dermatitis treatments. The case highlights the importance of considering DRIF and performing skin biopsies in patients with persistent papular rashes.
