TLDR Combining low-dose dutasteride with finasteride improved a man's hair loss.
The document reports a case where a 52-year-old male patient with androgenetic alopecia (AGA) experienced improvement in hair loss after combining low-dose dutasteride with finasteride. Initially, the patient had been taking 1 mg of finasteride daily for four years, which led to improvement after 18 months, but no further improvement was observed after 34 months, and hair loss progression was noted. Subsequently, the patient was given an additional 0.5 mg of dutasteride once a week, which resulted in improvement after 7 months. The patient maintained this combined therapy for approximately 1 year and 4 months without any significant side effects observed in blood tests or clinically. The document discusses the role of 5α-reductase (5AR) inhibitors in treating AGA, noting that finasteride inhibits type 2 and possibly type 3 5AR, while dutasteride inhibits both type 1 and type 2 5AR more strongly. The combination therapy was chosen to enhance the inhibition of type 1 5AR while maintaining the effects on type 2 and 3 5AR. The document concludes that more research is needed to fully understand the role of type 3 5AR and the efficacy of combination therapy in AGA.
