This case report discusses a 50-year-old male with idiopathic isolated adrenocorticotropic hormone (ACTH) deficiency (IAD) who presented with alopecia areata and a testicular germ cell tumor. This is the first known case of IAD initially manifesting as alopecia areata and being associated with a testicular germ cell tumor. The patient experienced symptoms such as hair loss, fatigue, and poor mental health. Treatment involved oral glucocorticoids and surgical resection of the tumor, leading to significant improvement in symptoms and normalization of hormone levels after 2 months. The report suggests a potential link between alopecia areata, IAD, and testicular germ cell tumors, possibly due to an autoimmune response from paraneoplastic syndrome, highlighting the need for thorough screening for neoplastic diseases in patients with unexplained IAD.
