This case report details a 50-year-old male with idiopathic isolated adrenocorticotropic hormone deficiency (IAD) and a testicular germ cell tumor, presenting with alopecia areata. The patient's alopecia worsened despite treatment, but improved after surgical removal of the tumor and hydrocortisone therapy, indicating a potential autoimmune link. The case is unique as it is the first to report IAD with both alopecia areata and a testicular germ cell tumor, suggesting a possible paraneoplastic syndrome. The report highlights the importance of considering alopecia areata in IAD diagnosis and the need for comprehensive neoplastic screening in adult-onset IAD cases. It suggests that germ cell tumors may be an underrecognized factor in adult IAD, calling for further research into the genetic and mechanistic links between IAD and neoplastic conditions.
